You follow all the standards, do the tests, and carefully write the instructions. Everything looks clean on paper. But once it hits the market, support tickets pile up for things you thought were impossible. The classic misuse appears. A component is installed upside down. A vent gets blocked. A device is left outside, in the rain.

And someone on the team says:

“But they weren’t supposed to use it like that.”

Exactly. And that’s the problem.

The industry often treats misuse like an exception, an edge case. Something unpredictable. But reality says otherwise: misuse is common, predictable, and often entirely foreseeable. Standards say it too. But our risk analysis files? They usually don’t.

This article dives into why misuse must be treated as a normal condition in risk analysis. It explains how to address it correctly. Acknowledging misuse early can protect your users and your business.

Surely there will never be a product that cannot be used in such a “creative” way that can cause harm. Nevertheless, designing a safe product shall consider the possibility of a misuse.
Misuse is part of every product life cycle, just like misunderstanding happens even to the best communicators.

Most product safety standards include a clear requirement to document and mitigate reasonably foreseeable misuse. It’s not a “nice to have”, it’s mandatory.

Here are just a few examples:

  • IEC 61010-1 (Safety requirements for electrical equipment for measurement, control, and laboratory use) Has an entire paragraph dedicated to reasonable foreseeable misuse
  • ISO 12100 (Machinery safety – General principles for design): “All phases of the machine life cycle and all reasonably foreseeable misuse must be included in the hazard identification process.”
  • ISO 14971 (Risk management for medical devices): Requires systematic identification of risks arising from “reasonably foreseeable misuse.”

These clauses are often overlooked in practice. levering the fact that the true idiot proof product cannot be created. Risk files include ideal use cases, but skip the messier reality of what users actually do.

Defining “Reasonably Foreseeable Misuse”

Here comes the tricky part, the general critique is that this classification is too vague and is prone to interpretation. The key word is “reasonable.” Not every misuse is avoidable, but we must focus misuse that a manufacturer can reasonably expect based on:

  • Common misunderstandings of product design

The shift in global regulations (like the transition from the MDD to the MDR in the medical world, or the update to the Machinery Directive) has placed a much higher burden of proof on the manufacturer. You must prove that you performed “Usability Validation”, essentially, watching real people use your product and documenting their errors.

To stay ahead of the regulatory curve, your risk file should include:

  • User Personas: Clearly define who the user is. Is it a professional with 10 years of experience, or a consumer with none?
  • Use Environment: Is it a quiet office, or a loud, vibrating construction site? Environment dictates the likelihood of lapses and slips.
  • Residual Risk Justification: For every misuse you can’t design out, you must provide a strong justification for why the remaining risk is acceptable.

This proactive stance doesn’t just pass audits; it builds a brand reputation for reliability. When a product is “intuitive,” it’s because the engineers did the hard work of anticipating and neutralizing human error before it ever happened.


The Psychology of the “User Error”

For example, a user may assume a device is “off” because the screen is dark. In reality, it is still energized.

If your product requires a user to push a handle to save themselves, you have a fundamental design flaw that no risk file can truly “label” away.

4. Documentation Fatigue

Risk files are seen as a formality. Check the box, copy-paste from last time, and move on. This leads to generic tables that don’t reflect real use.

Field Data: The Reality Check for Your Risk File

The greatest enemy of an accurate risk analysis is the “Vacuum.” This is when a team of engineers sits in a quiet conference room and tries to imagine what might go wrong. This approach is limited by the team’s own expertise and biases. To find the “real” misuse, you need to examine the data from the field. It often tells a story far stranger than anything you could imagine in a boardroom.

Customer support logs are the most underutilized tool in compliance. If three customers have called in because they accidentally broke a plastic latch, that latch is no longer “sufficiently strong” for foreseeable use. It doesn’t matter if it passed the lab’s “static load test”; it failed the “real-world user test.”

When updating your risk files, create a feedback loop with these departments:

  • Field Service: Ask them what parts they replace most often. Frequent replacements usually indicate a design that is being stressed in ways you didn’t intend.
